Payment and insurance

I am a provider for most insurance plans, including Anthem Blue Cross, Blue Shield, Aetna, United Behavioral Health, Magellan, Cigna, and others. For out-of-network insurance plans, service may be covered in full or in part by your health insurance or employee benefit plan.

Payment

Payment or co-payment is due in full at the time services are rendered. Cash and check payments are accepted; credit cards are not.

Cancellation Policy

If you do not show up for your scheduled therapy appointment and you have not notified me at least 24 hours in advance, you may be required to pay the full cost of the session.
